Federico Caselli 34e6b732a1 Fetch first support ...

Add support to ``FETCH {FIRST | NEXT} [ count ] {ROW | ROWS}
 {ONLY | WITH TIES}`` in the select for the supported backends,
currently PostgreSQL, Oracle and MSSQL.
